Patient Notice Regarding Vendor Breach

Carle Health is deeply committed to protecting the security and confidentiality of our patients’ information. This notice is to inform our CCIN patients about an incident involving some of that information.

On April 29, 2021, Elekta, a data storage vendor, confirmed with Carle Health that they experienced a security incident on various dates between April 2 and April 20, 2021. Immediately upon learning of this incident, Elekta launched a detailed investigation and contacted law enforcement. On April 22, 2021, their investigation confirmed that an unauthorized user gained access to Elekta’s system containing protected health information from Carle Cancer Institute Normal “CCIN.” On May 26, 2021, Elekta shared the results of their investigation with CCIN. At this time, Elekta has no indication information was disclosed publicly or used for any other fraudulent purpose.

In working with Elekta, the following types of patient information may have been involved in the incident: full name, social security number, address, demographics, date of birth, height, weight, medical diagnosis, medical treatment details, and appointment confirmations.

Carle Health and CCIN encourage anyone receiving a notification letter to take advantage of the credit monitoring and identity protection services offered by the vendor. We also urge those affected to remain vigilant and consider monitoring statements from financial institutions for any suspicious transactions and to contact the issuing institution if they see any activity they do not recognize.

Anyone may request their credit report once per year at no cost and receive information about fraud alerts and security freezes, via the official Federal Trade Commission web site found at www.annualcreditreport.com or by calling 1-877- 322-8228. You may also fill out a request form at https://www.annualcreditreport.com/manualRequestForm.action, and mail it to Annual Credit Report Request Service P.O. Box 105281, Atlanta, GA 30348-5281.
